Welcome to Centennial Middle School: How to Register A New Student

1. Use the following link to begin the process to register a NEW STUDENT:
   1. go to https://grades.provo.edu and select ‘New Student Registration’
      • click the link to Pre-Register your student
   2. After receiving email confirmation, follow directions in the email to fill out the online forms for your student
2. Provide the following to the school:
   • Birth Certificate: (wallet size not acceptable)
   • Immunization records: (or Utah State Exemption form)
     • Fill out state required ‘Pink Card’
   • Proof of residency with guardian’s name and address proving that you live in Centennial boundaries. This must be one of the following:
     • a current rental or mortgage agreement or
     • a current utility bill (gas, power, or city)
   • Withdrawal Form from previous school (if registering during school year)
   • Current copy of ‘IEP’ (if applicable)
3. Fill out Elective Choice Form
4. Fill out Emergency Contact Form: (form provided by school)
5. Show Photo ID
6. Pay School Fees or fill out ‘Fee Exemption Waiver’

(If you are living with another family, you will need to go to the Provo School District Office, Student Services building, to get a document from them)
